(Note: For modification or cancellation of appointment, contact Mrs Priyanka P.S 48 hours before the booked appointment. Otherwise, the session fee paid will be considered as cancellation fee.)
Step 4: Once the payment is done, it is mandatory to fill this form :- https://www.unwindcentral.com/pre-assessment-intake-and-consent-form/ before your session begins.
